We are providers for majority of PPO plans and some
HMO plans.
For most
HMO plans a referral is required either from the insurance company or the primary dentist. A pre-authorization is required
for most HMO plans before treatment is rendered.
We will make every effort to assure you receive maximum benefits
from your insurance company, and we will file claims for you out of courtesy. We ask that you read your policy to be sure
that you are fully aware of any limitations of the benefits provided. Your policy may base its allowances on a fixed
rate fee schedule that may or may not coincide with our usual fees.
We will give you a breakdown of all fees and costs before treatment is administered.
Unless you have made a special arrangement, payment is expected at the time treatment is administered. For your convenience,
we accept most major Credit Cards.
Facts About Dental Insurance:
Dental insurance is a contract between your employer and a dental insurance
company. The benefits that you will receive are based on the terms of the contract that were negotiated between your employer
and the dental insurance company and not your dental office. The goal of most dental insurance policies is to provide only
basic care for specific dental services. The services selected are based on the cost of the policy to your employer and the
negotiated arrangements with the dental insurance company. Because the benefits you currently have are decided between your
employer and the insurance company, some services are not covered. The selection of non-covered services is not based on what
you need or want but is based strictly on the contract with the insurance company.
